In focused physical examination OSCEs, you have to examine the requested
body part or system. No head to toe examination. But, if the complete
examination of a system require examining some other body parts, then it is
included.
For example, a complete cardiovascular examination will include
examination of the legs for peripheral pulses and edema as well as
opthalmoscopy for cardiac related changes among others.

You have to explain what you will be doing to the patient taking
necessary permission before you proceed. Pay attention to patient privacy
and draping. Don't harm or repeat harmful maneuvers.
If an examiner is present in the room, please remember to explain every
thing you are doing to the examiner and stand in a way to let them watch
you. Don't stand in between the examiner and the patient.
