A Step By Step Guide To Mastering The OSCE

And Patient Clinical Encounters

Don't you find it difficult to find a way to ORGANIZE the OSCE Exam station to achieve a smooth going cooperative patient interview?... Do you forget WHAT to ask and do during your short OSCE Exam Station because of your nervousness?...  Have you thought of HOW to ask history questions and perform physical examinations correctly meeting clinical and communication guidelines?...  WHEN to ask all the REQUIRED questions, do each part of the physical examination in a time efficient sequence while communicating effectively and fulfilling the OSCE exam checklist at the same time?...   This proven TIME Efficient OSCEhome Systematic Approach may be the solution you are looking for...

OSCEhome Systematic Approach flowcharts help you PUT yourself on an autopilot mode during OSCE exams enabling you to focus on the differentials, clinical decision making, and your performance.

No need to think WHAT to ask or do, HOW, and WHEN ! ...

OSCEhome Systematic Approach

Patients visit doctors complaining of a symptom, not a disease. It is all about the differential diagnosis of symptoms and signs. This is the main focus of physicians during patients’ interviews, right?

OSCE stations, as well as real life patients’ interviews, have limited time.  Physicians have no choice but to be focused and to organize the interview in a time efficient manner. On the other hand, physicians have to be vigilant not to miss anything!

Obviously, there is a need for some sort of an approach that will fulfil both patients and physicians goals. An approach that will explore all the patients’ presenting issues in a limited time frame. An approach that will protect both the patients and physicians.

How did we at OSCEhome formulated the OSCEhome Systematic Approach ?

We started by preparing a list of all signs and symptoms a physician faces. We wrote one sign or symptom on a separate sheet of paper. Then for each symptom or sign we wrote all the possible differentials. Then, we draw a table, assigned a column for each differential and wrote that symptom/sign presentation details, quality, duration, relation to other symptoms/signs, and red flags.. Etc.. Then for every detail, we wrote questions to ask for or points to examine.

Obviously, there are now, many identical questions or points to examine concerning this symptom among all these differential diseases. We started to merge the diseases’ columns into one set of questions. When the same question is required for several diseases, we placed these diseases’ names between brackets after the question to help us later with the clinical decision making process. Then we arranged these questions and points to examine in a logical easy flowing flowchart.

We have noticed that there are some details that are not required if you are just screening for a symptom and not thoroughly gathering details about it. So, we divided the to ask/do list into two files, we placed the must ask/do important ones up in the list for screening, and the rest at the bottom for detailed data gathering sub-file, and just in case we run out of time.

What about communication skills? It a major issue to ensure an easygoing interview professional organized interview and to achieve a mutual understanding and respect. We rephrased the questions to meet communication skills guidelines. e.g. English language issues, open ended questions, non-leading questions, respective manner ..etc.

Now, we have all the needed questions to ask and points to examine concerning that symptom on a separate sheet of paper. We placed the sheet in a separate file, labelled it with the symptom/sign name. Then, we repeated the same process for all other signs and symptoms.  Again, there are now, many identical questions or points to examine among all these symptom/signs.

We started to gather identical questions or points to examine into a separate file box and crossed it out on that symptom/ sign sheet. e.g questions about medications, past illnesses, social, ..etc.

Now, we have three sets of box files;

  1. Chief complaint data gathering box file,

  2. Specific symptom/sign box files, and

  3. Standard questions box file.

Remember, in real practice, as well as during OSCE stations, our time is limited. So, we carefully went into each of these box files and rearranged the questions and points to examine for maximum time efficiency.

We came up with a tree of a step by step history taking and physical file boxes. During the patient interview, you go through the file boxes one by one. You only open the box files you need.

Let us take an example. A patient presented with cough.

  1. Introductions box: 5 sentences to say.

  2. Chief Complaint: 10 question to ask.

  3. HPI: 15 questions to ask.

  4. Respiratory question box: 10 questions to ask.

  5. Standard questions box: 14 questions to ask.

  6. Wrap up box: Sentences for 8 points to explain.

DONE!

Most of the patients’ answers will be “NO”. How long then, will it take you to ask all these questions and wrap up the case?

FIVE minutes! And you have COVERED all the guidelines and checklists.

We created similar flowcharts for the physical examination, counselling, and ER stations that, with practice, will take you just 5 minutes to perform.

ONE flow chart of 7 steps with 23 history taking and 24 physical examinations boxes to choose one depending on the case !

Don’t you agree, it is much more easier to memorize than 488+ checklists!

Rest assured that all the guidelines and checks in ANY checklist are fulfilled, including communication skills.

You won’t forget anything to ask, do, or explain. No need to anxious and nervous.

You will be on a relaxed autopilot mode! Letting you focusing on the clinical decision process and communication skills.

The systematic approach took three of us a full year to finalize and eight years so far to tweak.

Don’t you agree, it is worth a try? It is yours in just few minutes.

Do all your patient clinical interviews get the results you really hope? Are they as successful as you'd like them to be?...

Do you frequently forget to ask or examine relevant points and have to go back to complete your history taking or physical examination?..

Do you frequently use phrases like "patient is a poor historian/ unreliable" or have communication issues while your colleagues don't think so?..

Even if your clinical and communication skills are doing alright – wouldn't you like to increase your efficiency substantially, even by as much as 3 times?

One of my OSCE candidates did just that. Using what I'm about to show you, he increased his time efficiency as well as his clinical skills substantially. Many others increased their clinical performance by 300% or more, as well.

Read what he emailed to me:

Dear Dr. Al Imari:

I didn't know about you when I made my first attempt at the OSCE. I failed in exactly the same way you describe except I didn't get frustrated or angry. I have knowledge. I just was not organized or focused.

Where were you when I needed you? I am going to be doing the exam a second time because I did exactly all the things you say happen when you take this exam without specific preparation. You know everything but I failed to find you. Tanking up on coffee beforehand does not help!

I am impressed with this material. I have two books with checklists. What a bunch of crap to burden your brain with. You have the information I need, the others were waste of time and money. If I cannot keep my focus with the help of your material then nothing will help. Now that I am retaking the exam I googled for a way to prepare for this exam.

I found you just in time. I expect to go into the exam in an organized fashion thanks to your material. You see when I went to medical school in Germany 30 years ago and then when I practiced medicine I always looked at the whole patient and that takes too much time. Time you do not have in the exam.

Sincere regards

George Butlers.

* Who am I, what are my clinical skills problems?

But let us first tell you who am I! Chances are you haven’t heard of OSCEhome before. But when you finish reading this, you’ll be glad you finally did.

I think I’m a lot like you. I was in the same place you where not too long time ago. I’ve gone to medical school. . Spend my life going to school, seem like. I’ve run up these huge debts. I had to move to another city with all the hassle and expenses of settling down and figuring out my way. Medical school was long and expensive. But I’ve done really really well in everything I tried.

Unfortunately, for the first time of my life I was intern with basically no money. It was a challenge that is really not well defined and, you know, was sort of thrown to the wall and I have to figure out a lot of things. I’ll be honest. I had a tremendous fear of failure because all along through school it was clear for me what to do to have success.

But now, all of a sudden I have to worry about all kinds of things, like how to deal with patients, how to take history and do physical, how to be fast and yet thorough, how to make the right decision in a short time, how to deal with nurses and staff. And doing better than my peers in case presentation, exam evaluations, and being a ‘good doctor’. Some of these guys, I think, like nothing more than seeing me humiliated in front of them, nurses, staff, and everyone.

There is that whole idea of being out there all of a sudden alone. What would I do? The Hippocratic oath said I’m not supposed to hurt anyone or cause harm, and definitely I don’t want to do that. I don’t want to miss things or get patient complaints or law suits. So, where to start?

In my patient interviews, I was always nervous and in a hurry. Didn’t know the pace I was going. It took me a long time to clarify patient’s complaint and HPI. Wasn’t able to finish asking all question in one time or forgot the patient’s answers, always needed to go back and clarify things and thus my patient notes were deficient.

In my clinical skills evaluation exams, the picture wasn’t much different. Adrenaline was high. Patients were of all different kinds. Angry, sad, seductive, talkative and so on. The one I still remember was a patient who kept pacing the room rapidly talking none stop and at the end left the room! Patients who ask stupid questions like scratch marks. Angry patients who takes time to calm them down. I always wished to know a quick way to deal with such difficult patients.

My physical examination was all over the room, not well organized. Takes time, slow patients, and again forgot things to check or looking after the patient’s comfort like sliding the bed. I remember once I was so nervous that I used the tooth pick to assess touch sensation which hurt the patient (she moaned). I apologized, but still looked nervous and don’t know how to do the test.

My patient notes were not that good too. My hand writing was shaky. HPI not complete and not in sequence as it should be.

As you see, I was basically where you are now when I had all these fears. I had a great success in my scholastic studies in med school. But here, now all of a sudden I was making almost very little money with this crushing debt burden beside all the emotional stresses and I really have to perform. But it was all milky. I didn’t know what and how to perform.

When we started our clinical rotation in medical school, we, to some extend, left by our own to learn clinical skills. When we make mistakes, we'll always be able to go back and check with the patient or reexamine. The patient was like a mountain that I have find out our path to climb to reach the dome, that is the diagnosis. I was, and still am, afraid of missing something that will lead to harm the patient or upset him, or make him complain to my attendant affecting my assessment, or even sue me.

Most of the time, patients were supportive. Well, they know I am just a medical student or junior resident trying to learn. Besides, I avoided challenging patients. But this is not the case in real life medical encounters. I haven't enough time to have a relaxed interview pace or even to recheck with the patient. When I was preparing for our Clinical Skills Assessment Medical OSCE Exam, I, just like you, had spent weeks looking for that “Clinical Skills Assessment Medical OSCE Exam magic approach” that could allow us to finally master a thorough and yet time efficient clinical and communication skills approach.

* How did I solved my clinical skills & communication skills problems?

I virtually attended all available expensive Clinical Skills preparation courses, bought and studied almost all available preparation books. I listened to every ‘Clinical Skills Assessment Medical OSCE Exam Expert’ who offered advice. I spent so much time practicing. But NOTHING WORKED as I hoped! That is to be thorough, organized, and time efficient at the same time and off course good communicators!

Then, four years ago, on a brilliantly sunny day in October, while studying at the central library, I came across few Medical OSCE examiners. They were getting ready for an OSCE exam. I chatted with them and discovered the COLD HARD TRUTH about why I couldn’t reach any significant level of confidence in how thorough and quick is my Clinical Skills Assessment Medical OSCE Exam Clinical Skills approach! Most probably, you are now in a similar stressful situation.

Hello:

I bought this ebook from you last night, it is amazing... I could not stop reading that book.... I am taking one preparatory class for QE2 in Toronto... now I feel that if I had this book before I would not need to go for that class... this book is the best book for preparing for OSCE....

The TRUTH about OSCE Exams that I discovered is that my scattered OSCE Exam preparation information, notes, questions, and tips, NEED TO BE gathered in ONE medically thorough, organized, workable, and time efficient step by step  SYSTEMATIC APPROACH flowcharts that integrates BOTH clinical skills and communication skills that I can study and practice ONCE for any patient scenario and for ALL my coming OSCE Exams as well as my FUTURE real life medical practice with real patients! An approach that get me high clinical evaluation and scores now, and patient satisfaction avoiding complaints and law suits later!

All OSCE Exam stations' formats are essentially the same in all OSCE exams. The differences between Medical School Students OSCEs, USMLE Step 2 CS, LMCC MCCQE IIPLAB part 2MRCGP CSA,  TRAS 2,  AMC Clinical,  NZREX, or Clinical Skills Assessment of Foreign / International Medical Graduatess are in the time allocated to the OSCE stations, the actual presence of the OSCE Examiner during the OSCE station, and the depth of needed medical knowledge, that is the OSCE Exam checklists!

They also told me that the OSCE station's stem question is just to direct the OSCE Exam candidate to what system I should focus on giving the limited available time but not meant to omit the differentials and hidden issues. Checklists approach won't get you a high clinical evaluation score!

* So What Did I Do To Avoid Missing Things And Waste The interview's short time?

Over a really long time, I gathered all the necessary information needed to be asked for every disease. Then I:

  • Arranged them in question boxes based on symptom complaints.

  • Wrote/gathered specific questions for each needed info.

  • Checked the medical, ethical, and communication correctness of each question with experts.

  • Determined the differentials for each symptom and merged the necessary questions within the boxes.

  • Arranged the questions within each box in a time efficient flowchart sequence that assures patient cooperation and prevent them from interrupting the medical interview flow.

  • I subdivided each box into a main screening box and detailed sub-boxes to be explored only if the patient reply was related.

So, In a real OSCE, as well as a medical interview, I'll start with the main first question box and follow through expanding into sub-boxes as needed and the back to the main box. Then, according to the station, move to the next box and so on. It took me about a year to perfect the system.

For the physical exam, I:

  • Arranged the needed exams to be done in a time efficient sequence that minimize patient discomfort.

  • Wrote phrases to ask the patient to perform a test or move in a clear, ethical and communication correct manner that prevents patient interruptions to the exam flow.

  • Wrote explaining phrases to say to the patient while performing the steps to keep them informed.

I did the same for counseling stations, patients note, and how to deal with difficult patients.

After going through all these fears, frustration, and the aspiration to figure out this solution. After fighting all that and getting the hard knowledge. I sat down and realized that there is a great need among other medical students, interns, residents, as well as international foreign medical graduates out there to have this information.

So, what I did I put together  this website, OSCEhome.

* Do you need OSCEhome step by step systematic approach to be clinically efficient??

If I could give you an effective Clinical Skills strategy that would “outperform” the medical interview approach you are currently using, would you be interested?

Let me explain...

You didn't figure out how to drive by yourself! .. Yes?!.. In stead, you started with some one who knows how to drive well and learned from him/her those specific driving steps, short cuts, and valuable secrets that you now use them automatically unconsciously every day!

In your preparation to your OSCE exam, don't you realize that it is noticeably logical to take the same approach to shape up your communication and clinical skills?!

When you got your driving lessons, your instructor asked you to follow specific steps and perform specific procedures in a specific sequence,... right?!.. That's because these specific steps and its sequence have proved to be the most QUICK, logical, and safe practices. By strictly following these steps, you can insure your safety, the safety of other road users, and, of course, passing successfully the driving license test!.

In OSCE exams, and subsequently in your real life medical encounters with real patients, OSCE examiners want you to be organized by following specific steps in specific medically and ethically correct sequence in order to be considered a thorough, efficient and safe physician.

That is a healthy approach and I sincerely encourage you to do the same in your Clinical Skills Assessment Medical OSCE exam preparation! Find out the specific steps and the specific sequence! That was the approach that I took!... I developed a step by step system. Verified it with my staff physicians. Memorized it. Practiced it. Modified and improved it over and over with the help of my mentors, faculty, and friends. It took OVER ONE YEAR of effort I don't know how many medical students, residents, and graduates helped to develop this advanced system! It was kind of an open project.

Finally, I was satisfied that I had created the best and the most comprehensive system of managing Clinical Skills Assessment Medical OSCE Exam stations and real life medical patient interviews possible and ready to start using it.

* And, was my clinical systematic approach ever a hit?!..

  • As each one in my group has his or her own contacts with other medical students, graduates and residents, the system started to spread rapidly!..

  • More and more medical students, graduates and residents started to contact us to send them a copy!

  • They loved our Step By Step systematic approach!

  • They loved the straight forward simple yet comprehensive questions system that manages any medical OSCE station or encounter!

  • They loved how I organized the questions to be asked easily in an autopilot mode!

  • They loved that the verbal communications skills are covered unconsciously!

  • They loved that the system prevents the patient from surprises us with arguments!

  • I received comments like “That’s awesome,…. Now, I feel confident,…. You guys really found the secrets!…. It saves a lot of time!….”

Many felt the increased confidence in their clinical skills and communication skills in less than a month!

* And that’s when it hit me!

Once I realized just how efficient it is to follow this systematic approach and saw how popular our Step By Step Guide became with other medical students, graduates and residents achieving high scores. I knew I’d be crazy not to make this system available to everyone who wants to master the clinical skills and pass the Clinical Skills Assessment Medical OSCE Exams.

When you start preparing for your Clinical Skills Assessment Medical OSCE Exam, what you need is a resource that doesn’t just give you copies of the same old checklists/  mark sheets everyone else has! Or, a limited time membership access to those checklists and videos which are designed to inform you WHAT should you do and you have to figure out HOW!! A 20-30 minutes videos that will show you how to do a 5 minute physical exam!! They took 20-30 minutes to show you the correct way and you have to find a way to do it in 5 minutes in your OSCE Exam!! Do you get it?!! Cannot you read that from any illustrated textbook or free online university websites?!! You don't need that.

You need a system that does the job in 5-15 minutes! Just memorize it, practice it, and that's it! Not figuring out how!

What you need is a real system that works. A system that covers:

Testimonials

I had the most enjoyable time preparing for my exam using your absolutely useful tools for passing the exam, and I passed. Thank you for all the help and effort that you have made to help residents in their oral exams.

Dana N.

My OSCE was last Friday 24/10/08. You will be pleased to know that I did well and really must say a massive big thank you. Your help was absolutely wonderful. I passed on your advice to some of my colleagues that were interested and they think it was great too. Thanks once again.

Crystal  H.

I have purchased some other examination books which are extremely comprehensive but don't add dialogue - combining such with your text would mean I would only have to purchase one book and not have to make notes - which is good for me. Communication and Hx taking are mentioned in the crash course range and the differentials are good - however they don't give a structured approach to Hx taking as your book does - which I like.

From OSCEhome survey.

After finding you on the internet at OSCE home you quite exactly described how I felt taking the exam. I figured if you could describe how the exam went for me you would have the answer as to how I could do better and not get disoriented, disorganized and lose focus.

From OSCEhome survey.
